Polypharmacy, defined as regular use of at least five medications, is common in older adults, but it can sometimes increase the risk of adverse medical outcomes. Explore how to define what medications we should consider discontinuing in the elderly patient, when this intervention should be considered, and how to approach these conversations with our providers.
